Well Child Visits

To keep your children healthy, it is important to bring them in for regular checkups.  Well visits include a variety of assessments, including, but not limited to:  measuring growth; checking for anemia; checking vision and hearing; screening for high cholesterol; checking vaccine status; and checking developmental status and emotional health and well being.

To measure developmental status and emotional health and well being, we utilize a HIPPA compliant online service named CHADIS.  CHADIS questionnaires are available online and need to be completed prior to your child's scheduled appointment.  For children age 12 years and up, both a parent and child questionnaire need to be completed in advance.  In addition, because we feel the well child exam is one of the most important visits your child has, we do require a parent (not babysitter/nanny/other relative) to accompany the child to the office.
