To help provide timely service to our patients, we prefer to schedule all visits.
For chronic illness complaints, we offer visits within a week.
If you feel that more than one child needs to be seen, please tell us when you make the appointment, so that we can schedule accordingly.
We offer:
We require a current immunization certificate and a summary of care, as a minimum, prior to seeing a new patient for a well child visit. We will see new patients for sick visits with only a current immunization record.
If you arrive more than 15 minutes for your appointment, we will try to work you back into the schedule, but it may be necessary to wait until after those who arrive on time are seen.
To keep your children healthy, it is important to bring them in for regular checkups. Well visits include assessments of:
Because we feel the well child exam is one of the most important visits your child has, we do require a parent to accompany the child to the office, and not a babysitter, nanny, or other relative.
To measure developmental status and emotional health and well being, we use a HIPPA-compliant online service named CHADIS. CHADIS questionnaires are available online and need to be completed prior to your child’s scheduled appointment. For children age 12 years and up, both a parent and child questionnaire need to be completed.
Learn more about CHADIS »We make every effort to schedule appointments for ill children on the day that you call us. Please call our office ahead of time so that we can minimize the time that you will spend waiting. If you have a medical emergency during office hours, please call 911. For non-emergency visits, we will do our best to see your child immediately or refer you to the appropriate facility.
If you would like us to see more than one child, please schedule all of them when you make your appointment. Without an appointment, we may not be able to see a sick sibling right away, and you may be asked to wait while the provider sees a scheduled patient who may be waiting.
If your child’s illness or injury is life-threatening, call 911.
Emergencies are considered to be those conditions that are life-threatening: loss of consciousness, severe bleeding, seizure, etc.
In the case of cuts, mild fractures, sprains, etc., please call our office first for instructions.
Dr. Warner takes calls from her home after hours. Call the main office number, and the answering service will page her to call you back.
You can also reach her using Anytime Pediatrics. Please use Google Chrome as your internet browser when using this service.
Please save routine questions (mild illness, appointments, prescription refills) for office hours. We encourage you to utilize our patient portal My Kid’s Chart for these requests.
If a member of your family requires hospitalization, we have privileges at:
Cincinnati Children’s Hospital
3333 Burnet Avenue
Cincinnati, OH 45229
(513) 636-4200
Because your child deserves the undivided attention of a full-time hospitalist if he/she is ill enough to require admission, we do not admit our own patients.
After the admission, please follow your insurance company requirements to reduce your risk of payment problems.
At Union Pediatrics:
The recommended vaccines and their schedule are the result of years and years of scientific study and research, with data gathered on millions of children by thousands of our brightest scientists and physicians.
Therefore, if you plan on refusing, or have chosen to refuse, vaccinating your child, we are not the right office for you. As medical professionals, we feel strongly that vaccinating children on schedule with currently available vaccines is absolutely the right thing to do for all children and young adults.
Vaccination resources »Effective 2/3/20, we will no longer be sending out paper billing statements in the mail. All statements will be available on your patient portal. If you would rather receive a paper bill, there will be a $2.00 surcharge.
Due to the increasing number of high deductible health plans, effective 2/3/20, we will be instituting an Auto-Pay policy. Please review our financial policy for more details.
Please bring your insurance card with you to each visit.
If your insurance has changed, we will notify you in advance of your appointment, and ask that you provide your new insurance information to us. We require your insurance information in advance so that we can verify coverage and establish if any deductible amount will be due at the time of your visit.
Copays and deductibles are due at the time of service.
We have contracts with most insurance carriers, but always recommend you call the insurance company to verify we are an in-network physician for your particular plan.
With regard to KY Medicaid, we are no longer accepting new patients from any of the plans.
For your convenience, you can download and print these PDF forms and complete them at home, prior to your visit.
